California has stood up its own version of the federal Rural Health Transformation Program (RHTP) the California Rural Health Transformation (CalRHT) program, and it’s now accepting applications for its first wave of grants. For rural clinics and health centers exploring how to fund vaccine storage and inventory automation, CalRHT is worth understanding closely. It’s also a program where the details matter: not every category of CalRHT funding covers equipment the way you might expect. Here’s an accurate breakdown.
What CalRHT Is
CalRHT is California’s implementation of the federal RHTP, led by the Department of Health Care Access and Information (HCAI) with CMS oversight. California was awarded $233.6 million for Federal Fiscal Year 2026, and the program is structured around three initiatives: Transformative Care Model, Workforce Development, and Technology & Tools. Grant opportunities are being released in phases, with new programs announced over time.
What’s Open Right Now
Four CalRHT programs are accepting applications:
- Accelerator Partners (Transformative Care Model): funding for hospitals and rural organizations implementing regional care collaboratives and hub-and-spoke models, with an emphasis on strengthening primary and maternity care metrics.
- Expand and Support Rural Workforce Capacity / FM-OB Fellowship: funding for graduate medical education institutions expanding family medicine obstetrics fellowship capacity.
- Workforce Development Recruitment and Retention (WDRR): funding to recruit, relocate, or retain health professionals at qualifying rural sites.
- EHR Modernization Grants (Technology & Tools): funding limited specifically to adopting a first-time EHR or replacing an existing one.
The Important Nuance: Not All Categories Cover Equipment
This is where CalRHT differs meaningfully from what some other states have published, and it’s worth being precise about. Capital expenditures and general infrastructure or equipment upgrades are explicitly not an allowable use of funds for Accelerator Partners — CMS’s Category J (“Capital Expenditures and Infrastructure”) is specifically excluded for this program. So an application framed simply as “purchase this equipment” would not fit.
However, that’s not the end of the story. CMS’s federal allowable-use categories that are permitted under Accelerator Partners include:
- Category C — Consumer tech solutions, covering technology-driven tools for chronic disease prevention and management
- Category D — Training and technical assistance, covering the adoption of technology-enabled care solutions including remote monitoring and advanced technologies
- Category F — IT advances, covering technical assistance, software, and hardware for significant information technology advances that improve efficiency and patient health outcomes
That distinction matters for automated vaccine storage and inventory technology. A system like AccuVax or AccuShelf isn’t just refrigeration hardware, it’s a connected technology platform with automated compliance monitoring, real-time data logging, and remote reporting. Positioned correctly, that kind of technology-enabled solution may fit within Category F.

- Review the CalRHT Grant Funding Opportunities page and sign up for the Rural Health News and Updates mailing list, since additional Technology & Tools grants are expected to open in future phases.
- Contact the CalRHT program team at info@calruralhealth.org or 855-CAL-RHTP (855-225-7487) to confirm whether your specific vaccine storage or inventory technology project would be classified under an allowable use category before you apply.
- If applying under Accelerator Partners, build your narrative around the technology-enablement and care-access angle, not an equipment-purchase framing, and be prepared to show alignment with at least one Transformative Care Model metric.
- Watch for future infrastructure-focused CalRHT releases. Grant categories more directly aligned with facility equipment purchases may open in later phases as the program’s remaining Technology & Tools and Transformative Care Model funding is released.
